Service Orders
Service Orders are requests which normally require maintenance beyond the occupant’s Self-Help capability. These jobs are accomplished on a priority basis 1, 2, or 3. The Government has established specific criteria by which priorities are determined and the priorities are assigned by our Customer Service and Work Reception Office at the time of your call.
Please, be as descriptive and specific of the problem as possible so the allocation of time and material can be Properly identified, logged and passed on to the maintenance personnel.
All work is accomplished during normal duty days. Appointments are scheduled in advance in order to allow you adequate time to arrange your personal schedule.
Changes, or simply breaking an appointment lead to confusion and loss of productivity. We try our best to be on time, please allow 1/2 hour leeway on either side of your scheduled appointment. Please note that all repairs must be requested on a Service Order.

Real Property Accountable officers, tenant units and occupants are responsible to make minor repairs. The Self Help Issue Point (SHIP) enables occupants to make minor repairs and improvements to their building and reduces the amount of backlog.

Directorate of Public Works, Work Order DA Form 4283
All requests beyond the scope of service orders must be submitted on a DA Form 4283 Facilities Engineering Work Request updated September 2003.
All work performed on government controlled, or in government facilities must be submitted for approval on this form (one original).
The requester completes the upper portion of DA Form 4283 (part "A" except customer ID and document serial number which will filled in by DPW Work Reception) in accordance with the instructions on the back of the form.
All work requests must be signed by the Organizations Real Property Responsible Officer (RO) in the block Authorized Requestor/Authorized Requestor Signature.
The signed original needs to be emailed to:

The requester will receive an email with the work order number.
